Terms and Conditions of Membership
1. Professional Members. Any person shall be eligible to be a Professional Member
if he/she:
a. Is a physician, non-physician (radiation therapists, physicists, etc.), health
care professional or scientist in good standing; and
b. maintains a direct role in CyberKnife patient care or CyberKnife related
research, and is affiliated with a contracted or installed CyberKnife site; and
c. has earned an academic degree that reflects medical training. Scientists
shall have an equivalent degree (e.g. PhD, EngScD, etc); and
d. satisfies such other qualifications as may be adopted or amended by the
board from time to time.
2. Dues, Fees, and Assessments. Each Professional Member must pay, within the
time and on the conditions set by the Board, the dues, fees, and assessments in
amounts to be fixed annually by the Board. The dues, fees, and assessments shall
be equal for all Professional Members. Dues, fees and assessments are non-
refundable.
3. Good Standing. Those Professional Members who have paid the required dues,
fees, and assessments in accordance with these bylaws and who are not
suspended shall be Professional Members in good standing.
4. Termination and Suspension of Membership.
a. Causes of Termination. The membership of a Professional Member shall
terminate on occurrence of any of the following events:
b. Resignation of a Professional Member by written notice to the Society;
c. Expiration of the period of membership, unless the membership is renewed
on the renewal terms fixed by the Board;
d. Failure of a Professional Member to pay dues, fees, or assessments as set
by the Board within sixty (60) days after they become due and payable;
e. Occurrence of any event that renders a Professional Member ineligible for
membership, or failure to continue to satisfy the membership qualifications.
5. Transfer of Memberships. Individual Professional Memberships are not
transferable. Institutional Memberships maybe be transferred but the total
number of memberships may not exceed five (5) active persons at any given time
during the ten (10) year period.
